 school year with Project LINK's 2026 Back-to-School Health Clinics! These c
 linics provide school physicals, routine immunizations, Florida Form DH 680
  certificates, and immunization record transfers for eligible students. Ser
 vices are available for children ages 5–17 who are uninsured or enrolled in
  Medicaid, including students entering kindergarten, 7th grade, or enrollin
 g in Hillsborough County Public Schools for the first time.<br /><br />Appo
 intments are required and can be scheduled on the website or through the Ba
 ck2School Clinic Scheduler mobile app. Please do not visit a clinic locatio
 n without a confirmed appointment. To help your visit go smoothly, have you
 r child's current immunization records ready and upload them during registr
 ation if possible. Your child must be present for all appointment types, in
 cluding DH 680 updates or transfers. There is a one-time $10 registration f
 ee per family, regardless of how many children are registered.<br /><br />C
 linics will be held on Saturdays from July 11 through August 15, 2026. Regi
 ster early to reserve your spot and make sure your child is healthy, protec
