
APPOINTMENTS ARE REQUIRED
Get your child ready for a successful school year with Project LINK's 2026 Back-to-School Health Clinics! These clinics provide school physicals, routine immunizations, Florida Form DH 680 certificates, and immunization record transfers for eligible students. Services are available for children ages 5–17 who are uninsured or enrolled in Medicaid, including students entering kindergarten, 7th grade, or enrolling in Hillsborough County Public Schools for the first time.
Appointments are required and can be scheduled on the website or through the Back2School Clinic Scheduler mobile app. Please do not visit a clinic location without a confirmed appointment. To help your visit go smoothly, have your child's current immunization records ready and upload them during registration if possible. Your child must be present for all appointment types, including DH 680 updates or transfers. There is a one-time $10 registration fee per family, regardless of how many children are registered.
Clinics will be held on Saturdays from July 11 through August 15, 2026. Register early to reserve your spot and make sure your child is healthy, protected, and ready for the first day of school!
